Dear Dr. Bernstein:
I am very pleased to have your kind letter of December 1. I have been hoping to see your brother-in-law, Frank Westheimer, again, but circumstances have caused me to make several trips through Chicago during the last year or two without having had time enough to get in touch with him.
I am very much interested to learn about the patient who has periodic attacks of anemia with marked sickling, at the age of 31 or 32. In our investigations we have not as yet made any study of precipitating factors for sickle cell crises. It seems to me that it would be very good to know whether the patient that you mention has the amount of sickle cell hemoglobin in her blood corresponding to sickle cell anemia or just to sickle cell trait. Perhaps she is heterozygous, but abnormal with respect to the amount of sickle anemia hemoglobin that she manufactures. Most of the heterozygous individuals that we have investigated showed about 60 percent of normal hemoglobin and 40 percent of sickle cell hemoglobin in their corpuscles; but it seems to me not outside the realm of probability that some individuals with sickle cell trait would manufacture larger amounts of sickle cell anemia hemoglobin.
We could make a study of this point if it were possible to get a sample of blood - about 20 ml - from this patient. It wouldn't be necessary that the blood be taken after a period of several months during which she had not received any transfusions. Do you think that this could be arranged?
