Video Reserve Request Form

Instructors may place a "hold" on library Videos or DVD´s to be used in the classroom later in the term. Please make requests at least 2 weeks prior to date(s) needed. This will ensure the video/DVD will be available on your requested date(s). Videos/DVD´s are checked out to instructors for three (3) days, or as needed. The videos/DVD´s will be available at the Circulation counter on the requested date. Please ask for this video by the call number when you come to pick it up.
