Course Reserves Request Form

  • Please fill out completely; incomplete forms take longer to process. Please allow ONE WEEK for processing. You will be notified via email when the items are on reserve. Items will be removed at the end of the designated term; personal items will be sent to the instructor´s campus address, and library items will be returned to the shelf.
  • Articles, assignments, solutions, class notes, tests, etc. will be put in 3-ring binders provided by the library.
  • For classes with more than 20 students, we highly recommend having at least 2 copies of required materials.
  • Blackboard has many practical alternatives to Course Reserves. Click on "FAQ´s" at http://my.oregonstate.edu/ to learn more.

All copyrighted placed on reserve must conform to copyright law. A signature is required to show compliance of this law. Forms for placing copyrighted materials on reserve are available at Circulation or here for download.

If you would like to save a copy of your submitted form to use again in the future, do the following:

  1. After you have submitted the form you will see a page showing what you have just submitted to course reserves. Save this page in a location you can access in the future. (Choose File > Save As in your browser). The document will be saved as an HTML file.
  2. When you would like to revise (term, year, course, and add new items) and resubmit the same form, open the file in a word processing program (ie. MS Word), make your revisions, save and attach the HTML file to an email to "valley.reserves@oregonstate.edu".

Course Reserve Stamp
In order to prevent theft or attempts to sell your personal books, the Valley Library uses a rubber stamp to mark these items for Course Reserves. The library is not responsible for personal reserve items that are lost or damaged.
