Circulation Request Form

Use this form to:

  • request an article/item from another location (Cascades, Guin, Storage)
  • place a trace on an available item which could not be found in the stacks
  • rush an item with a status of "in process" or "in repair"

If an item has a status of "in bindery" or "on order", check the Summit Catalog or Interlibrary Loan to see if you can request the item.

You can place a hold on a checked out Valley, Cascades or Guin item or an available Storage item yourself by going to the OSU Libraries Catalog, locating the item, and placing a "request" on the item from another library.

Please fill out as many fields as possible. Complete requests have better chances of being filled quickly and successfully. Incomplete requests may delay processing or be returned to you.
